How To Join

Because we feel it is important to know you personally, you and your family are invited to make an appointment for a free, no-obligation visit to our office to meet with the physician. Just call 360-438-1161 or you can email us and ask for information about joining our practice.


Our Fee Schedule (Effective September 1, 2008)

Primary Fees:
Adult (18 and over) $95 per month
Couple $170 per month
Child under age 18 or full-time dependent student* $45 per month
VP Special Care (house calls only) $205 per month
Suboxone Treatment $150 per month

Discounts:
5% Annual Payment Discount Given for annual payment made in full at time of enrollment.
VP Family Plan This membership includes families of 4 or more and is priced at a rate of $260 per month.

Please Note: Enrollment requires minimum 3 month commitment.

*Membership for children requires at least one adult primary membership, unless special written authorization is provided by Vantage Physicians.
